Music Speaks Notice of Privacy Practice (NPP)

This notice describes how medical information about the client may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment to your privacy

Music Speaks is dedicated to maintaining the privacy of the client’s personal and health information as part of providing professional care. The music therapist will maintain confidentiality of client documentation, information, records, and communication as required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and state and federal laws. A copy of the more extensive version of this law is available upon request. This abbreviated writing of the law serves to fulfill the legal requirement to notify you of privacy practices. How Music Speaks uses and discloses your protected health information with your consent

We will use the information collected about your loved one to provide the necessary treatment. After you have read this notice, it is requested you sign your understanding of this information within the Service Agreement form. Treatment cannot be provided if the form is not initialed and signed.

 

Disclosing your health information without your consent

There are some times when the law requires us to use or share your information. For example:

  1. When there is a serious threat to the client’s or another’s health and safety or to the public. We will only share information with persons who are able to help prevent or reduce the threat.

  2. When we are required to do so by lawsuits and other legal or court proceedings.

  3. If a law enforcement official requires us to do so.

  4. For workers’ compensation and similar benefit programs.

 

Your rights regarding your health information

  1. You can ask the therapist to communicate with you in a particular way or at a certain place that is more private for you. For example, you can ask the therapist to call you at home, and not at work, to schedule or cancel an appointment.

  2. You can ask the therapist to limit communication with others regarding treatment, such as to other family members and friends.

  3. You have the right to look at the health information Music Speaks has on file about the client, including documentation and progress notes.

  4. You have the right to a copy of this notice. If there is a change in notice, you will be notified of the changes and receive a written copy of the changes.

  5. You have the right to file a complaint if you believe your privacy rights have been violated. Filing a complaint will not change the health care provided to you in any way.

You may have other rights that are granted to you by the laws of your state, and these may be the same as or different from the rights described above. Please speak with a Music Speaks representative to discuss and questions or concerns.
